Please provide a computer-generated flight itinerary from the airline or a travel agency.

U.S. Visa or Green Card Requirement

Notarized copy of U.S. Permanent Resident Card or valid long-term U.S. visa (for non-U.S. citizens)

PASSPORT SIZE PHOTOS

Submit two recent passport-sized photos. In-house photo service available for $15. You can also visit Walgreens, FedEx Office, or CVS. Follow official photo guidelines for approval.
