One (1) signed visa application form.                                                                                                                                          The Consulate of Vietnam requires that the visa application be Completed Online. The form is best accessed using Explorer, Firefox, or Chrome.  Be patient as load times can be protracted.  Finally, the form will open in Vietnamese.  You must click “English”, located in the upper-right corner of the page, to complete the form in English. Your completed application must:

• Be completed in 15 minutes or fewer
• Be signed in blue or black ink in the lower right portion of the form, underneath the date you completed the application

Notes:

Application preparation or rewrite will be charged additional fees on site*

• Please make sure that your name is spelled exactly the same as the name appearing on the relevant ID or document. No variations are permitted.

• An original signature of the applicant on the signature line on the application form. Please make sure that your signature is on the line, and that the signature must match the signature in the applicants passport.

• Every question on the application must be answered, answer NONE where it does not apply to you. 

• Applications must be typed in BLOCK CAPITAL LETTERS; handwritten changes are not accepted. on the applicant’s passport. 

• DOUBLE SIDED APPLICATIONS ARE NOT ACCEPTED.  .
