1. Letter from Employer
A formal letter from the applicant’s employer must be printed on official company letterhead and addressed to:

The letter should include the following:
- Applicant’s position in the company and duration of employment
- Detailed purpose and nature of the visit to Japan
- Daily schedule or itinerary during the stay in Japan
- Business contact in Japan: Include full name, company name, address, phone number, and designated contact person
- Responsibility for expenses: Clearly state who will be covering all costs incurred during the trip
Important: The letter must be signed in wet ink by a company official (no electronic signatures accepted).
2. Invitation Letter from Japan
- A formal invitation letter from the host company or business contact in Japan

- A signed Letter of Authorization authorizing the agency or representative to submit on your behalf

4. Proof of U.S. Residence
Submit one of the following showing current address:
- Driver’s License
- State I.D.
- Recent Utility Bill
5. Immigration Documents (Non-U.S. Citizens Only)
- Original Permanent Resident Card (Green Card) or valid U.S. Resident Permit
- For F or J visa holders: Submit a copy of your I-20 or DS-2019 with a valid signature from an appropriate school/program official
Photocopies are acceptable.
