Copy of US Permanent Residence Card, Student ID Card, or a Valid U.S Work Visa

Original letter from employer or sponsoring company

Take two (2) copies of your passport sized photographs. See guidelines below

Copy of confirmed hotel confirmation

Original Version (NOT COPY)

Must be valid for the next (6) six months

Have at least two blank visa pages back to back

Not be frayed, torn, separating, or altered in any other way

Print & Sign completed application

Forward the confirmation to [email protected]

An original valid certificate of immunization against yellow fever is required to process Ghana visa.
